Kyogle Pharmacy, in partnership with the Wellness Partners Foundation, is seeking a passionate pharmacist to join our growing team in the beautiful Northern Rivers region of New South Wales.

This unique conjoint role combines traditional community pharmacy practice with meaningful aged care, outreach and Equity Care services delivered across Kyogle and surrounding communities.

You'll have the opportunity to develop advanced clinical skills, build genuine relationships with patients, and help improve healthcare access for older Australians and vulnerable people living in regional communities.

Why this role is different

This isn't a corporate pharmacy role focused solely on script numbers and retail KPIs.

You'll work across both community pharmacy and pharmacist-led outreach services, delivering healthcare where it's needed most.

Your work may include:

• Community pharmacy services across Kyogle's two local pharmacies

• Residential aged care facilities

• Home care and community care services

• Home Medicines Reviews (HMRs)

• Residential Medication Management Reviews (RMMRs)

• Rural outreach and Equity Care programs

• Preventative health and healthy ageing initiatives

• Wellness Partners Foundation community programs

Working alongside GPs, nurses, aged care providers and community organisations, you'll help improve health outcomes across one of Australia's most beautiful regional areas.

What you'll do:

• Deliver high-quality community pharmacy services within two retail settings

• Conduct HMRs and RMMRs

• Support medication adherence and health literacy

• Identify and resolve medication-related problems

• Assist with deprescribing and medication optimisation

• Work collaboratively with local healthcare teams

• Deliver pharmacist-led care directly within aged care settings

• Support vulnerable and underserved populations through Equity Care initiatives

• Contribute to preventative health and healthy ageing programs
